REGISTRATION : Please complete a separate on-line registration form for EACH child.  Registrations and payment are due by Tuesday, August 20, 2024.  The sacramental education registration fee for each child is $60.00.   Registrations will not be taken after the deadline.  PLEASE NOTE:  Payment must be received prior to the start of classes.

First Reconciliation & First Holy Communion Religious Education Registration Form

Use this form for registration for FIRST HOLY COMMUNION only.

TIME OF CLASS:  First Holy Communion Classes are held on Sunday mornings from 10:00am to 11:15am (limit of 11 students -CLOSED as of 7/23/24 as it is filled to capacity) at the Main Church classrooms and Tuesday evening from 6:00pm to 7:30pm at the SMDLP Social Hall.

First Holy Communion normally takes place in May.  Once we are closer to the date, we will send out  additional information regarding specific dates and times for First Holy Communion

CIRCLE OF GRACE:

This special class will be taught as part of your child's regular religious education.  This class aims to teach children how to be safe, protect themselves, and communicate any potential danger. This instruction will take place during the regular Religious Education class times during the month of September 2024. The materials that will be used for this instruction will be available for you to view in the office of the Director of Religious Education.

If for any reason, you DO NOT  WANT  your child  to participate in this class, SIGN THE FORM UPLOAD IT WITH THIS SUBMISSION.

PHOTO AND LIBRARY RELEASE:

Please download and read in its entirety the Archdiocese of Santa Fe Authorization Form for use of child/youth name, likeness, and/or photographic image and for use of our Queen of Peace library.  Checking the box below shall serve as parental permission for the use of name, likeness, and/or photographic image of child/youth where such permission is required and to be entered into our Library database to be issued a library card.
